But the US is not an empire. I fail to see how you can compare that against the Roman Empire from over 2000 years ago.

The Roman Empire was ruled by an emperor, not a king.

Does it simply follow that all Empires be ruled by an Emperor?

No. For example the English Empire was ruled by a King or Queen.
Both an Emperor/Empress and a King/Queen are Monarchs.
However, Emperor/Empress is considered the higher Monarchy titles.

The main difference is an Emperor/Empress has no recognition of superiority of another leader. while a King/Queen can.
An Emperor/Empress would never pay tribute to another ruler because they are considered above any other.
A good example of this was the Emperor of Japan during WWII. He was considered to be above everyone else, and was never prosecuted or even indicted for war crimes.

Hirohito was considered a God by the Japanese. He was not prosecuted because it was never clear just how involved he was in the decision making, militarily speaking. This had nothing whatsoever to do with his title. Regardless the title bestowed it boils down to their ability to lead their own and subjugate others, title notwithstanding. It comes down to the damage you can inflict and just how much damage the rulers country can withstand and their ability to answer any aggression. I will see your Hirohito and raise you a Margaret Thatcher.
America has a good win/loss record in conflict, not because the fight any better than anyone else but rather the homelands ability to keep the military healthy and completely supplied with anything they need and more. Germany would have/should have won the first and second world wars but the United States ability to repair and replace men, equipment and supplies was and still is second to none. Neither Japan nor Germany had the resources to wage a world war. The U.S. has a lowly President way below any of your well-heeled door men and women.

It was very clear what he did and how involved he was. But nobody was going to go there because he was the Emperor of Japan.
It would be the same as trying to get the Pope to court, Nobody is going to go there.
